Company Contact Changes Form

The EEO-1 Component 1 Filer Support Team is available to assist you. Complete the form below to submit your company change. Once processed, the new point of contact will automatically receive the company ID and passcode via an automated email. NOTE: Company changes are to be completed by a representative from the Parent Company. Establishments are not authorized to make company changes.

Please be advised that due to the large number of company contact changes, the response time may be longer than usual. However, we will ensure that filers will be able to file and that no one will be excluded. We also ask that companies do not submit the same form multiple times since doing so results in further help desk delays.
